Step 1: Complete Your Profile Information

  2. Update Details: Click on Current Information (as shown in the attached image) and update all necessary details.
  3. Patient History: Complete the Patient History section, as it is the most important part.

  • Telehealth Services Informed Consent
  • Evaluation & Treatment Consent Form
  • Insurance & Payment Form – Completing this is critical to avoid billing issues.
  • Adult ADHD Symptom Rating Scale – Observer Version (ASRS-O)
  • Bipolar Spectrum Diagnostic Scale (BSD)
  • The Mood Disorder Questionnaire
