Medi-Cal covers Spravato through your managed care plan’s pharmacy or medical benefit, depending on the plan. Either way:
Medi-Cal does not cover intravenous or intramuscular ketamine for depression, anxiety, or PTSD, since those uses are off-label. Spravato is FDA-approved for treatment-resistant depression, which is why it is the ketamine-route option Medi-Cal actually reimburses.

The Spravato manufacturer savings program is limited to patients with commercial insurance. Patients with Medi-Cal or other government coverage are not eligible for that program. Your cost-sharing is instead set by your Medi-Cal managed care plan.
