Spravato is governed by an FDA Risk Evaluation and Mitigation Strategy, or REMS, which is why treatment looks different from picking up a prescription. Every dose is self-administered in a certified clinic under direct observation, followed by at least 2 hours of monitoring, and you cannot drive until the following day. This article explains what those rules are, why they exist, and what they mean for your schedule.
Three things must be enrolled and active before a first dose: the treatment setting, the prescriber, and the patient. The medication is shipped to the certified clinic, never to a pharmacy for home use. During and after each dose, staff monitor blood pressure, sedation, and dissociation at intervals, and document a discharge check before you leave. The FDA modified the REMS program most recently in late 2025, but the core setting, observation, and driving requirements remain.
Esketamine can cause transient dissociation, sedation, and blood pressure elevation, and these effects peak within the first 2 hours. Monitoring means the uncommon problem is caught in a clinical setting rather than at home or behind the wheel. Most people feel back to baseline before they leave.
Plan on roughly 2.5 hours per visit and arrange a ride home every time. The labeled schedule is twice weekly during weeks 1 to 4, weekly during weeks 5 to 8, and weekly or every 2 weeks in maintenance depending on response.
